Pharmacy – Specialty Prescriptions
Some self-medications self-administered specialty drugs, such as injectable, infused, oral or inhaled drugs must be purchased from a participating specialty pharmacy. Florida Blue offers you two choices: Accredo Specialty or CVS Caremark.
Pharmacy – Retail & Delivery
Prime Therapeutics offers a network of thousands of participating pharmacies. Using one of these pharmacies and choosing generic medicines when possible will maximize your savings. Out-of-network pharmacy expenses are not covered.

Delta Dental DeltaCare USA Plan
Under this option, you select a Primary Care Provider from the DeltaCare USA provider list. No claim forms to complete. No copays for basic cleanings.
Delta Dental PPO Option (Indemnity)
Under this option, you can receive services from a PPO Dentist or the dentist of your choice. PPO Dentists will file claims on your behalf and have agreed to charge no more than the predetermined PPO fee schedule. You may be required to pay up-front costs and file a claim form if you use a non-Delta Dental dentist.
